Sample collection

Take note of the following when collecting specimen:

Blood specimen:

  • Only use K2EDTA tubes for blood collection
  • Each blood tube must be labeled with at least 2 identifiers (i.e., Name and NRIC)
  • Ensure that the completed test request form, consent form (if necessary) is attached with the specimen
  • Blood specimen collected at closing hours and only sending the following day, store the sample at 2-8°C overnight.

 

FFPE tissue specimen:

  • FFPE tissue block or unstained tissue slides in a sturdy box or container
  • Ensure that the completed test request form, consent form (if necessary) is attached with the specimen
  • Test Request Form to be completed as below:
    1. Patient’s details including full name, hospital registration number (RN), sex, date of birth, age, identification number, race, contact no.
    2. Requesting physician name, signature, MMC registration no., referring hospital/clinic/lab, email address and date
    3. Relevant clinical history including diagnosis, stage of cancer, treatment history
    4. Test is selected (🗸)
    5. Date of request
