Privacy Policy - Singapore
Introduction
Pathomics Health is committed to ensuring the appropriate protection of all information assets within Pathomics Health’s possession, in accordance with Singapore Data Protection Act (Act 26 of 2012) (“PDPA”), the Healthcare Services Act (HCSA), the Ministry of Health’s (MOH) guidelines, directives and any other relevant legislation.
Personal data refers to data about an individual who can be identified from that data, or from that data and other information to which we have or are likely to have access.
This Personal Data Protection Policy explains how Pathomics Health collect, use, store, disclose and protect all personal information. Furthermore, it describes the type of information collected, as well as the reasons and methods of accessing and correcting such information.
The policy herein serves as a supplement and therefore does not supersede or act as a substitute to any other prior consent that have been provided to Pathomics Health regarding personal data provided to us. Pathomics Health also reserves all rights granted by the law to collect, use, or disclose personal data.
Type of Information Collected
Pathomics Health may be provided, or collect personal data which include but not limited to:
Name;
NRIC Number, Passport Number, or any other identification number;
Gender;
Date of Birth;
Nationality;
Personal Contact Number;
Email Address;
Residential Address;
Any relevant personal information provided to Pathomics Health.
Collection of Personal Data
Personal data may be provided to or collected by Pathomics Health in any of the following circumstances:
when an account is created or registered with us, and/or submission of any form such as laboratory test request form to us.
when specimens or any other organic materials is provided to us in a manner that is associated with data which could identify an individual.
when personal data is disclosed during interaction with our staff which includes Customer Service Officers, Medical Technologists, Phlebotomists and any other Pathomics Health Representatives via face-to-face meetings, telephone calls (which may be recorded), emails, letters and fax.
when visiting the premises of Pathomics Health, images are captured by our CCTVs, or when attending events hosted or sponsored by Pathomics Health during which photographs, and videos are taken by Pathomics Health representatives.
when services are undertaken through online platforms, such as Pathomics Health’s website, including any other online platforms, technologies, or tools.
when there is a request to contact the individual, or to be included in email or other mailing list in connection with updates to Pathomics Health’s services.
when the individual respond to Pathomics Health’s request for additional personal data for promotional purposes or other initiatives.
when personal data is obtained from referral parties, public agencies, employer, or any other relevant third party.
when personal data is obtained for the purpose of facilitating payment process.
when browsing Pathomics Health websites, information may be automatically captured by our web server (i.e., cookies) for the purpose of enhancing browsing experience.
when the personal data is provided to Pathomics Health for any other purposes.
Purpose for the Collection and Use of Personal Data
Pathomics Health collects and uses the personal data for the purpose of:
verifying identity of individual;
responding to, handling, and processing queries, requests, applications, complaints, and feedback from individual;
enabling the provision of Pathomics Health’s services to third party who are interested in our services;
effectively administering, managing, monitoring and improving our services;
processing payment or credit transactions;
complying with quality assurance or clinical audit activities;
undertaking accreditation activities;
addressing liability indemnity arrangements and reporting;
managing and planning the administrative and business operations of Pathomics Health and complying with its internal policies and procedures;
complying with any applicable laws, regulations, codes of practice, guidelines, or rules, or to assist in law enforcement and investigations conducted by any governmental and/or regulatory authority;
preparing the defence for anticipated or existing legal proceedings;
undertaking research and the compilation or analysis of statistics relevant to public health and safety;
any other purposes which are reasonably related to the aforementioned.
By engaging Pathomics Health or any of our third parties for services, Pathomics Health may collect, retain, process, use disclose or transfer your personal data for the purposes listed above, unless otherwise notified by Pathomics Health.
Disclosure of Personal Data
By contacting or dealing with us, the individual herein agreed unilaterally that Pathomics Health may disclose an individual’s personal information to the following third parties which includes but not limited to:
other health service providers involved in the individual’s treatment or diagnostic services;
vendors or suppliers which Pathomics Health contract with authorized external parties or companies that provide products and services to us such as information technology security and support, payroll and employee expense support, and benefits and rewards administration;
relevant government ministries, regulatory bodies, statutory boards or authorities or law enforcement agencies to comply with any laws, rules, guidelines and regulations or directives imposed by any governmental authority;
professional advisors such as insurance companies, auditors and legal representatives;
any other party with reasonable purpose for the use of personal data.
Retention of Personal Data
Pathomics Health store personal and health information in both paper and electronic form.
The security of personal and health information is of utmost importance to us and Pathomics Health take reasonable steps to protect it from misuse, interference and loss and from unauthorised access, modification or disclosure.
Pathomics Health shall retain the personal data until the purpose for which that personal data was collected is no longer being served and retention is no longer necessary for legal or business purposes.
The period of retention of personal and health information is determined by Pathomics Health and/or stipulated by law after which it is de-identified or disposed of in a secure manner.
Accuracy of Personal Data
Pathomics Health takes all reasonable effort to ensure that personal data collected by or on behalf of us is accurate, complete and up-to-date, and ensure that personal data is relevant at the point of use or disclose.
The accuracy of that information depends largely on the quality of the information provided to us. For change and correction to personal data, the request will have to be made through the relevant third party (e.g., doctors), or when the relevant third party provides the updated information to us.
Accessing Personal Data
Pathomics Health shall, as soon as reasonably possible, provide the individual with personal data about the individual that is in the possession or under the control of the organisation.
If the situation does not permit us to provide access, Pathomics Health will provide alternative means of access or discuss how access can be given through a mutually agreed intermediary.
We will disclose the personal information and give access to the individual’s Authorized representative or legal adviser whom we have been given written authority to do so.
For personal data provided to us by a third party (e.g., doctors or employer), please be notified that any related query or complaint are to be made directly to the individual or organization directly.

Changes to the Policy
Pathomics Health reserves the right to review, change and update this policy from time to time to reflect our current practices and obligations and changes in technology. Pathomics Health will publish our current Personal Data Protection Policy on our website at www.pathomicshealth.com and the changes will take effect at the time of publishing.
It is advisable for individual to review this policy from time to time and remain familiar with its terms.
