Sample collection
Accuracy and reliability of test result depends largely on the specimen integrity, which could be affected by improper sample collection. The following are the points to take note when collecting specimen:
Using the correct type of blood tube for different tests;
All specimen tube must be accompanied by a test request form;
Each specimen tube must be labeled with at least 2 identifier (i.e., Name and NRIC/FIN/PP No.)
Test Request Form must have the following details:
Date of Request
Patient’s Full Name
Patient’s Identification Number (NRIC/FIN/PP No.)
Patient’s Date of Birth
Patient’s Gender
Date and Time of Specimen Collection
Specimen Type (i.e., Blood, Nasopharyngeal swab)
Requesting Clinic
Requesting Doctor Name and MCR No.
Test Code of Requested Test
Blood specimen collected at closing hours and only sending the following day, store the sample at 2-8°C overnight.
